I have a 2003 Astro, do I need to vacuum down my system to replace the ac pressure switch?

No. There's a seal to prevent that.

Would a bad pressure switch cause the ac clutch from disengaging? It doesn't disengage unless I turn the ac off.

I had to replace the switch on the accumulator on my '93 Bravada because the compressor wouldn't run until my husband jumpered the connector. That is the low pressure switch, though - and that's the switch I was thinking about when I answered you.

The high pressure switch is on the back of the compressor and I actually don't know if that requires you to recover the refrigerant first. I've never had to replace that one.
