Smooth Idle in Drive, Rough in 1st and 2nd, Engine noise

Hello, I have had my van since early this year and have already repaired or replaced a lot of parts in it. My issues for the longest time being I have a rough idle and a raw gas smell. The raw gas smell is now not as strong since my neighborhood mechanic fixed a vacuum leak. I also now get a smooth idle in Drive, but a somewhat rough idle in Reverse and Overdrive and a very rough idle in 1st and 2nd gear. When the idle is rough in 1st and 2nd gear the vibration comes primarily from the front middle part of the Van. Like around were the glove box, radio and space between the 2 front seats. I also still hear a vacuum leak type noise when starting the van up. Plus my heater/ ac controls don't not work but will work sometimes. To give you a very detailed description on what is wrong with my van I listed stuff that has already been done to her. As well as symptoms of what my van is doing. I have already spent $2,000 and would like to get down to the problem at hand. Any help would be gladly appreciated :) :)

Parts I have replaced and other tasks that have already been taken-

Radiator and T-Stat (Radiator had bad leak in it)
Radiator Cap
Fuel Pump (Fuel Pump failed so replaced with Delphi Fuel Pump from Napa)
Spark Plugs and Wires, Rotor, Fuel Filter and Air Filter
Drained old ATF and Put in new ATF
Replaced Fan Clutch (Was loose)
Repaired Vacuum Leak ( because of repairing this Vacuum Leak I no longer get a Rough Idle in Drive, my van also stopped having that gas smell. )
Have been putting Chevron 93 Octane in Van as well as tried a few bottles of Techron in gas. I also put Seafoam in gas for one fill-up

Problems I have been having-
Rough Idle
Gas Smell
Black Exhaust out tailpipe (The Tune-Up/ Fuel Pump replacement stopped that)
Whistling from engine as if I still have another vacuum leak
Air not coming through front vents (The vacuum leak fix has taken care of that)
Heater/ AC controls only sometimes would work (still have this issue)
Air from heater being cool (This may be a heater core issue so probably replacing that will solve this problem.)

I have also been running rich put no engine lights or codes have popped up. My vin number is Z and I have throttle body injection. My millage was 11-12 mpg prior to fuel pump replacement and tune-up. Now it seems I bounce around between 14-16 MPG. Will post my latest MPG number next time I put gas in her. My mechanic was also mentioning how a bad vacuum leak can fool the system into thinking your EGR valve is clogged. My O2 sensor also appears to been fine but might replace soon since I have no clue last time it was changed.

Looks like on my latest fill-up my MPG was 17.1. Looks as if fixing this Vacuum Leak has made an overall difference on how efficient my van is. I also replace a back tired that was getting cupped. Which stopped the rough idle in overdrive, reverse, 1st and 2nd gear. I still get a little bit of a front end vibration when moving slowly. However if isn't what it used to be so I'm not extremely worried. I also replaced my front and back end shocks today which stopped my ride from being as bouncy. They were 10% off at Advanced Auto and cost $20 a shock. While not perfect cheap shocks are better than bad shocks. (Which I had.)
